Minor & Young Traveler Consent Acknowledgement

Please review this acknowledgement carefully if you are traveling with someone under the age of 21 years old, especially if one or both parents or legal guardians are not traveling with them.

When traveling internationally with minors or young adults, additional documentation may be required if both parents or legal guardians are not traveling together.

​

Some countries, cruise lines, airlines, resorts, tour operators, or other travel suppliers may require a signed and notarized consent letter from the non-traveling parent or legal guardian. In some situations, proof of sole custody, guardianship, or other legal authorization may also be required.

For many destinations and travel suppliers, these requirements apply to travelers under the age of 18. However, some cruise lines, resorts, countries, or other travel suppliers may apply similar requirements to travelers who are 20 years old or younger.

A minor or young traveler consent form should generally include:

• Traveler’s full legal name

• Traveler’s date of birth

• Traveler’s passport number, if applicable

• Trip destination

• Travel dates

• Cruise line, airline, resort, or tour operator, if applicable

• Name of the adult traveling with the traveler

• Relationship of the traveling adult to the traveler

• Name and contact information of the non-traveling parent or legal guardian

• A clear statement giving permission for international travel

• Permission for emergency medical treatment, if desired or required

• Signature of the non-traveling parent or legal guardian

• Notary acknowledgment, if required or recommended

• Copy of the non-traveling parent or legal guardian’s photo identification, if required or recommended

Saltwater Sunsets Travel, LLC strongly recommends that travelers review all documentation requirements before travel and carry any required consent forms, birth certificates, custody documents, guardianship documents, passports, visas, travel authorizations, or other supporting documentation.

By proceeding with travel planning or booking, you acknowledge and understand that it is the traveler’s responsibility to confirm and obtain all required travel documentation for every traveler in the party. Requirements vary by country, cruise line, airline, resort, tour operator, and travel supplier.

You further acknowledge that failure to present required documentation may result in denied boarding, denied entry, cancellation of travel, or loss of travel funds.
